If you are a RENTING a home, you will need the following to establish service…
- Completed “Residential Service Forms”
- Two forms of identification (we are unable to accept military ID’s per the DoD)
- A copy of your signed lease agreement with your name and address.
- A deposit is required before service is established.
If you are a BUYING a home, you will need the following to establish service…
- Completed “Residential Service Forms”
- Two forms of identification (we are unable to accept military ID’s per the DoD)
- If the home is new construction, you will need a “Certificate of Occupancy”
- A deposit is required before service is established.
If you are RENTING a COMMERCIAL space, you will need the following to establish service…
- Completed “Commercial Service Forms”
- Two forms of identification (we are unable to accept military ID’s per the DoD)
- A copy of your business license with the City of Cabot.
- A copy of your signed lease agreement with your name or company name and address.
- A deposit is required before service is established.

In Person:
We are open Monday thru Friday from 8:00am until 4:30pm. If you need same day service you will need to be in our office located at #1 City Plaza, Suite B, Cabot, AR 72023, or have your information submitted online or by fax to us by 2:00pm.
Online:
For online service, scroll down to find the appropriate form for your service needs. The forms listed are PDF format and may be filled out online and emailed to customerservice@cabotwaterworks.com.

Service requests may also be faxed to 501-605-1743. If you choose to send your requests by fax please be sure to call us to make sure that it has been received.
All active accounts with Cabot WaterWorks are required to have a deposit. If you are a new customer or an existing customer that is moving to a new address that is serviced by Cabot WaterWorks, a deposit will have to be made before services are established.
All accounts will be required to provide two forms of identification, only one needs to be a photo ID. We will not accept active military ID's as we are not allowed to copy them per the DoD.
Renters, including commercial properties, will also be required to provide a copy of their lease agreement.
Meter deposits are based on meter size and type of service connection.
